8-3-2: APPLICATION; DEPOSIT AND INSPECTION FEE:
   (A)   Applications for permits shall be made in writing on a form provided by the City and submitted to the Superintendent of Public Works, or his or her assignee. It shall be accompanied by a detailed site plan detailing the work to be done and the dimensions and materials of the area to be excavated and such other information as may be required by the Superintendent of Public Works. Before any permit shall be issued, the applicant shall pay the applicable application and inspection fee and make a deposit with the City Comptroller, in the form of cash or check, together with an inspection fee, based upon the following:

   (B)   Any person making or causing to be made any excavation or opening for any purpose in any public right-of-way within the City shall cause the same to be repaired in the following manner:
      1.   The width of the trench shall be kept as narrow as the construction needs require. Proper bracing shall be maintained to prevent any collapse of adjoining ground; no tunnel shall exceed thirty six inches (36") in diameter except with proper bracing or shoring as approved by the Superintendent of Public Works.
      2.   All excavated material taken from the opening shall be removed from the location and the opening backfilled with sand and/or a fine aggregate (CA16), as approved by the Superintendent of Public Works. The backfill material shall be placed in lifts not to exceed twelve inches (12"). Each lift shall be thoroughly compacted before the next lift is placed.
      3.   Topsoil shall be replaced at a minimum depth of six inches (6"), and be properly graded, compacted, seeded or sodded. All seeded areas shall have a layer of straw or other form of germination blanket placed to facilitate growth. In addition, areas that are on a slope shall have erosion control blankets, and other erosion control measures (e.g., ditch checks and silt fencing) installed when deemed necessary by the Superintendent of Public Works.
      4.   Gravel surfaces shall be repaired with a thoroughly compacted aggregate (CA6) base equal to the thickness of the existing course, but in no case shall it be less than eight inches (8") in depth.
      5.   Bituminous and seal coat surfaces shall be repaired with a compacted hotmix asphalt (HMA) surface equal to the thickness of the existing surface, but in no case shall the compacted HMA be less than three inches (3") thick. For surfaces thicker than three inches (3"), it should be installed and compacted in lifts no thicker than three inches (3"). The base under the HMA shall be a compacted aggregate (CA6) base equal to the thickness of the existing base, but in no case shall it be less than eight inches (8") in depth. Unless otherwise directed, temporary repair with bituminous cold-patch will be allowed and maintained until such time as weather allows for the permanent patch using HMA.
      6.   Concrete surfaces shall be repaired with a concrete surface equal to the thickness of the existing slab, but in no case shall it be less than six inches (6") thick. The concrete surface shall be poured over a compacted aggregate (CA6) base with a thickness equal to the existing base, but in no case shall it be less than eight inches (8") in depth. The concrete surfacing shall be removed one foot (1') outside the perimeter of the trench opening. In excavating materials from the area to be patched, the perimeter of the patch must be saw cut as close to parallel and perpendicular to pavement edge and/or center line. The adjacent concrete pavement or concrete base (if applicable) shall be doweled to anchor the patch to the surrounding pavement. Unless otherwise directed, temporary repair with bituminous cold-patch will be allowed and maintained until such time as weather allows for the permanent patch using concrete.
      7.   Concrete curb and gutter shall be required with the dimensions matching the existing curb height and gutter width. The thickness of the flag shall be equal the depth of the existing flag thickness, but in no case shall it be less than nine inches (9") thick. The flag shall be poured over a compacted aggregate (CA6) base, with a minimum thickness of eight inches (8"). The adjacent curb and gutter shall be doweled on both ends with eighteen inch (18") dowel bars, placed at mid-depth, with one placed no closer than two inches (2") from the back of curb, and the second placed no closer than two inches (2") from the front face of the flag. Contraction joints shall be installed every fifteen feet (15').
   (C)   The application shall also show the date and dig number issued by JULIE (Illinois one-call system). Work shall not begin until such time as all public utilities have been marked or cleared.
   (D)   The failure to complete restoration work in a satisfactory manner may result in subsequent applications being denied, or a fifty percent (50%) increase in the cost of the required deposit. (Ord. 2210, 10-24-2017)
