A. City Clerk Responsibility: The city clerk shall be responsible for issuing the dog license required pursuant to this article.
B. Application: Applications for a dog license shall be upon a form provided by the city. The information required shall contain the following:
1. Name and address of the applicant;
2. Phone number of the applicant;
3. Description of the dog;
4. Proof of rabies vaccination; and
5. Proof of dog being spayed or neutered, if applicable. (Ord. 973, 2-20-1990)
C. License Fee: The license fees for dog licenses shall be set by city council resolution. (Ord. 0-07-1191, 10-10-2007)
