§ 31.057 MEMBERSHIP.
   (A)   General. Membership on the SJCLHCC shall consist of executive-level members (or their designees who have been granted decision-making powers). Each agency within the LHCC will have no more than one voting member. The SJCLHCC shall be governed and appointed by the Board of County Commissioners. This Committee includes a Chair, Co-Chair, PIT (Point In Time) Count Lead, Coordinated Entry Lead and a Secretary.
   (B)   Voting members.
      (1)   There shall be one voting member from each entity named below:
         (a)   South Eastern Association of Local Governments;
         (b)   San Juan Counseling;
         (c)   DWS;
         (d)   San Juan County Housing Authority;
         (e)   San Juan Health;
         (f)   San Juan Public Health;
         (g)   Utah Navajo Health;
         (h)   San Juan County School District;
         (I)   San Juan County Sheriff’s Department;
         (j)   Faith-based organizations;
         (k)   City of Blanding;
         (l)   City of Monticello;
         (m)   Town of Bluff;
         (n)   San Juan County Commissioner; and
         (o)   San Juan County Administration.
      (2)   Agencies wishing to be considered for voting membership on the SJCLHCC should submit an application to the executive committee.
   (C)   Meetings.
      (1)   The full SJCLHCC shall meet quarterly on the first Tuesday of the first month each month except July and December at 3:00 p.m. Meetings will be held at the Commission Chambers located at 117 South Main Street, Monticello, Utah.
      (2)   There shall be at least one-week notice that will contain a tentative agenda and minutes from the last meeting. A notice of 48 hours will be provided for emergency meetings. The first meeting of the calendar year shall be designated as the annual planning meeting, at which time elections are to occur. The last meeting of the calendar year will schedule meetings for the upcoming year.
   (D)   Attendance. All members are expected to regularly attend SJCLHCC meetings. The executive committee reserves the right to remove any member for consistent failure to attend.
   (E)   Quorum and voting.
      (1)   For funding decisions, a quorum consists of 15 voting members; all other decisions require eight voting members present.
      (2)   All votes will be taken by roll call or ballot of all official representatives. SJCLHCC members must attend 75% of the meetings during the preceding 12 months in order to be eligible to vote.
   (F)   Minutes of meetings. Recorded minutes shall be kept of every meeting and shall include, at a minimum, the date, time and place of the meeting, the number of the attending organization members, the topics discussed, the decisions reached and actions taken, the list of roll call votes on all motions, any reports made and other information as may be deemed necessary by the Chairperson. Minutes, agendas and meeting invites will be the responsibility of the Secretary.
