753.03 LICENSE APPLICATION; FEE.
   (a)   Any person desiring to operate a temporary store shall make written application to the City Manager on forms furnished by the City Manager. The fee for an application to operate a temporary store shall be one hundred dollars ($100.00) and shall be nonrefundable. Not later than thirty days after the filing of a completed application for a license, the applicant shall be notified by the City Manager of the decision on the issuance or denial of the license. Each application shall contain the following information:
(1)   The name and address of the owner of the goods to be sold.
(2)   A description of the place where such store is to be located and a site plan indicating where merchandise will be displayed and signs are to be located.
(3)   The nature and type of the goods to be offered for sale.
(4)   The day when the business of the temporary store will commence and the day it will end.
(5)   The name and home and business address of the operator.
(6)   A copy of any appropriate licenses required by the County Health Department pursuant to Ohio R.C. 3732.02 or may hereafter be amended.
(7)   Proof of authorization from the property owner of the property on which the temporary store is to be located for the applicant to operate a temporary store on said property.
(8)   Proof of registration with the City Division of Income Tax for income tax collection purposes.
   (b)   Additional information may be required by the City Manager for the review of such a license in order to provide and protect the public health, safety, and welfare.
