Application for a permit for the erection or relocation of a sign is made to the Code Administrator upon a form provided by the Code Administrator and must include the following information:
(A) Name and address of the owner of the sign;
(B) Street address or location of the property on which the sign is to be located, along with the name and address of the property owner;
(C) The type of sign or sign structure as defined in this chapter;
(D) A site plan with measurements showing the proposed location of the sign along with the locations and dimensions of all existing signs on the same premises;
(E) Specifications and drawings showing the materials, design, dimensions, structural supports and electrical components of the proposed sign;
(F) The written consent of the owner of the property, if the applicant is not the property owner;
(G) The name of the person erecting the sign, if not the applicant;
(H) A statement as to whether any electronic lights on the sign will be flashing or not; and
(I) A statement as to whether the sign will be single-faced, double-faced or multi-faced.
