§ 158.28 STORM WATER MANAGEMENT PLAN APPLICATION REQUIREMENTS.
   (A)   Application. The applicant must make a written application for approval of a Storm Water Management Plan by filing the application materials with the City Administrator/Clerk-Treasurer. If the applicant is also applying for a National Pollutant Discharge Elimination System (“NPDES”) permit, the NPDES permit application materials may be submitted to the city as the applicant’s Storm Water Management Plan application. However, the applicant must supplement the NPDES Permit application materials with any information required by this subchapter that is not included in the NPDES permit application. All drawings and information submitted must be clearly legible and printed in blue or black ink. The applicant must supply the city with two sets of all drawings and information.
   (B)   Application information. The application must include the following information:
      (1)   An application fee as set forth by § 36.02 of this code of ordinances;
      (2)   Required financial security;
      (3)   A Storm Water Management Plan. The Storm Water Management Plan must contain the following information:
         (a)   Existing site map. A map of the existing site conditions showing the site and the immediately adjacent properties which includes the following information:
            1.   The name and address of the applicant;
            2.   A legal description of the site;
            3.   A north point;
            4.   The date upon which the map was created;
            5.   The scale of the drawing. The drawing must be prepared to scale appropriate to the site of the project and suitable for the review to be performed;
            6.   The number of sheets;
            7.   An insert map at a scale sufficient to identify the location of the property and giving information such as the names of adjacent streets, railroads, utilities, subdivisions, cities, towns and other landmarks;
            8.   The existing topography of the site with a contour interval appropriate to the topography of the land but in no case having a contour interval greater than two feet;
            9.   A delineation of all streams, rivers, public waters and wetlands located on and immediately adjacent to the site. All off-site waters that will receive discharge must also be identified if they are within one-half mile of the site or if they are within one mile of the site and are designated as “special” or “impaired” waters by the state’s Pollution Control Agency;
            10.   The location and dimensions of any existing surface storm water drainage systems and the natural drainage patterns on and immediately adjacent to the site delineating in which direction and at what rate the surface storm water is conveyed from the site, identifying the receiving stream, river, public water or wetland and setting forth those areas of the unaltered site where the surface storm water collects; and
            11.   The location and description of any vegetative cover and a delineation of any vegetation proposed to be removed.
         (b)   Site construction plan. A site construction plan that includes the following information:
            1.   The location and dimensions of all proposed land disturbing activities and any phasing of those activities;
            2.   The locations and estimated dimensions of all temporary soil or dirt stockpiles;
            3.   The locations and dimensions of all construction site erosion control measures necessary to meet the requirements of this subchapter and the state;
            4.   A schedule of the anticipated start and completion dates of each land disturbing activity including the installation of construction site erosion control measures needed to meet the requirements of this subchapter; and
            5.   Provisions for maintenance of the construction site’s erosion control measures during the construction.
         (c)   Plan of final site conditions. A plan of final site conditions that is on the same scale as the existing site map that shows all changes to the site, including the following information:
            1.   The finished grading showing contours at the same interval as provided above or as required to clearly indicate the relationship of proposed changes to existing topography and remaining features;
            2.   A drainage plan of the developed site delineating in which direction and at what rate storm water will be conveyed from the site and setting forth the areas of the site where storm water will be allowed to collect;
            3.   The proposed size, alignment and intended use of any structures to be erected on the site;
            4.   A clear delineation and tabulation of all areas which will be paved or surfaced, including a description of the surfacing material to be used; and
            5.   Any other information pertinent to the particular project which, in the opinion of the city, is necessary for the review of the project.
