§ 155.21 REGISTRATION INFORMATION.
   (A)   Information required. The registrant must provide the following at the time of registration and promptly notify the city of changes in such information:
      (1)   Registrant’s name, address, telephone number, facsimile number and Gopher One Call registration certificate number if any;
      (2)   Name, street address and email address, telephone number and facsimile number of the registrant representative;
      (3)   (a)   A certificate of insurance from a company licensed to do business in the state providing coverage in at least the following amounts:
            1.   General liability. Public liability, including premises, products and complete operations, or other such amounts as shall be set by the City Council from time to time:
 
Bodily injury liability
$1,000,000 each person
$3,000,000 each occurrence
Property damage liability
$3,000,000 each occurrence
Bodily injury and property damage combined
$3,000,000 single limit
 
            2.   Comprehensive. Automobile liability insurance, including owned, non-owned and hired vehicles.
 
Bodily injury liability
$1,000,000 each person
Property damage liability
$3,000,000 each occurrence
In lieu of 1) and 2) bodily injury and property damage combined
$3,000,000 single limit
 
         (b)   1.   The certificate must verify that the registrant is insured against claims for personal injury, including death, as well as claims for property damage arising out of the:
               a.   Use and occupancy of the right-of-way by the registrant, its officers, agents, employees and permittees; and
               b.   Placement and use of equipment or facilities in the right-of-way by the registrant, its officers, agents, employees and permittees, including, but not limited to, protection against liability arising from completed operations, damage of underground equipment and collapse of property.
            2.   The certificate must also name the city as an additional insured as to whom the coverages required herein are in force and applicable and for whom defense will be provided as to all such coverages. The certificate must require that the city be notified 30 days prior to cancellation of the policy.
      (4)   24-hour emergency number;
      (5)   An acknowledgment by the registrant of the indemnification pursuant to § 155.60 of this chapter;
      (6)   Certificate of authority to do business in the state and in the city, if applicable; and
      (7)   Such other information as the Director may require.
   (B)   Notice of changes. The registrant must keep the information listed above current at all times by providing to the city information as to changes within 15 days following the date on which the registrant has knowledge of any change.
