A person who wishes to become a candidate for an elected office, shall complete and file a declaration of candidacy. The declaration shall be completed under oath before the City Clerk and on a form provided by the City Clerk. The declaration shall state definitely:
(a) The full name of the candidate and the manner in which he wishes his name to appear on the ballot;
(b) The full residence address of the candidate;
(c) The full mailing address of the candidate;
(d) The office for which the candidate declares;
(e) That the candidate is a qualified voter and resident of the city;
(f) That the candidate agrees to serve if elected to the office of Council member for a term of three years.
