Any person intending to operate a restaurant or food establishment shall, before opening or operating the establishment, make application for a permit to the county’s Health Officer upon a form provided by the Health Officer and shall contain the name of the owner or manager thereof, the location and such other information that the Health Officer may require for the administration of this chapter. The application shall further require a signed statement that the applicant will observe the provisions of this chapter.
