All applicants for a permit to operate a body art establishment shall submit to the Local Enforcement Agency all of the following:
(A) Completed application form.
(B) Establishment plans in sufficient detail to ascertain compliance with conditions in this chapter.
(C) An exposure control plan.
(D) Such other information as the LEA determines is necessary for implementation of this chapter; and
(E) Any applicable fees.
(F) Permits are valid for one year and shall be renewed annually.
(G) Permits must be prominently displayed within the body art establishment.
