(A) The application for such license shall contain the following information:
(1) The name, address, age, date, and place of birth, Social Security number, and business identification number of each person having a legal or equitable interest in a proprietorship, partnership, or corporation applying for a license;
(2) The place where each machine or device is to be displayed or operated and the business conducted at that place; and
(3) A description of each machine or device to be covered by the license, mechanical features, names of manufacturers, and serial number.
(B) No license shall be issued to any applicant unless he or she shall be over 21 years of age and a citizen of the United States.
