(A) The application for a certificate of appropriateness shall be made in the office of the Commission and submitted on an application form(s) provided by the Commission.
(B) All applications shall be subject to the rules and requirements, established by the Commission. The completed application form, including any application fee and supporting documentation as required by the Commission's rules, shall be submitted to the Commission in the office of the city designated to receive said applications. Supporting documentation is information in which the Commission requires to make a decision.
