115.07 REQUESTS BY PERSONS FOR ADVANCE NOTICE OF DISCUSSION OF SPECIFIC TYPES OF PUBLIC BUSINESS.
   (a)    Any person desiring advance notice of scheduled discussion of specific types of public business at regular or special meetings of Municipal bodies shall file with the Clerk a written request therefor specifying:
      (1)    The name of the person making the request.
      (2)    The Municipal body or bodies that the request pertains to.
      (3)    The specific type of public business that the request pertains to.
      (4)    The name and address of the person to whom written notice may be mailed, telegraphed or delivered.
      (5)    The name, address and telephone number of the person to whom oral or written notice may be delivered either during or outside of business hours.
      (6)    The number of calendar months which the request covers.
   (b)    Any such request shall be accompanied by stamped, self-addressed envelopes sufficient in number to cover the number of regular meetings during the time period covered by the request and an estimated number of special meetings. The Clerk shall use these envelopes to mail a written notice. The Clerk shall notify the requesting person in writing when the supply of envelopes is running out, and if the person desires notice after such supply has run out, he must deliver to the Clerk an additional, reasonable number of such envelopes as a condition to receiving further notifications.
   (c)    Any such request shall be effective for the number of calendar months specified in the request or until the supply runs out, or until the Clerk receives written notice cancelling or modifying such request, whichever occurs first. The Clerk shall so inform the person filing a request at the time it is filed. A person may modify or extend his request only by filing a complete new request with the Clerk.
   (d)    A request shall not be deemed to have been made unless it is complete in all respects.
   (e)    Requests hereunder may be conclusively relied on by the Clerk, Municipal body involved and the City.
