The application for a permit under this chapter shall give the exact location, the proposed opening, the kind of pavement, the size of the excavation and such other information as may be required. The permit shall not be issued until the applicant has given a cash deposit sufficient to cover the cost of restoration, which deposit shall be returned to the applicant upon the prompt and satisfactory restoration of all surfaces disturbed.
