Application for a license for public vehicles shall be made to the Director upon blanks provided by the Director and shall set forth:
(a) The name and address of the applicant;
(b) The tradename under which he does business;
(c) The class of vehicle for which he desires a license;
(d) The seating capacity;
(e) Any other information the Director may require.
If the license is granted, the applicant shall furnish his State license number and the length of time the vehicle has been in use, the motor number, body number, motor power thereof and the name and address of the person, firm or corporation from which such vehicle was purchased.
