The computer generated application form shall be prepared by the Department of Community Services and Development, Inspectional Services Division and shall require the name, address, and telephone number of the applicant, the particular type of work contemplated by the applicant, along with the permit number, date of expiration of the contractor’s or subcontractor’s state license, if any.
