§ 124.04 LIABILITY INSURANCE.
   (A)   (1)   All contractors and/or subcontractors receiving a permit as provided in this chapter are required to have a liability insurance policy providing coverage in the following amounts:
 
Liability - Bodily Injury
Liability - Property Damage
$100,000 per single instance
$300,000 for each occurrence
$25,000 minimum
 
      (2)   Evidence of the insurance shall be filed with the application to the Department of Community Services and Development, Inspectional Services Division in the form of a certificate of insurance naming the village as an additional insured under the policy. Each certificate shall have an endorsement to the effect that insurance may not be canceled by the insurer thereof without at least 30 days prior written notice to the village.
   (B)   Cancellation of the insurance provided for herein shall cause automatic revocation of the contractor’s or subcontractor’s permits.
