§ 111.16 PATRON RECORDS.

   (A)   Records of each patron shall be maintained for seven years.

   (B)   Each record shall include the following:

      (1)   Patron’s name;

      (2)   Address;

      (3)   Age;

      (4)   Date patron made decision to receive tattoo;

      (5)   Date tattooed;

      (6)   Design of the tattoo;

      (7)   Location of the tattoo on the patron’s body; and

      (8)   The name of the tattoo artist who performed the work.
