§ 113.16 APPLICATION FORMS.
   The forms used to apply for a license, or the renewal of a license, shall require the identity of the prospective licensee, the precise location of the building or outdoor location to be licensed, and such other information as is necessary to establish the eligibility of the applicant for a license.
