Application for a license or for renewal of a license to sell alcoholic liquor at retail shall be made in writing under oath on a form prescribed by the Commissioner. All applications for renewal of licenses must be submitted not less than 30 days prior to the expiration of the licensing period as herein established. The applications for Class D licenses shall be submitted to the Commissioner 14 calendar days prior to the date for which said license is to be in effect. The County Clerk shall prepare a sufficient quantity of the forms and shall provide the same to the applicant at no additional charge. Each applicant shall answer all questions on the application form.
