§ 95.127 APPLICATION FOR PERMIT.
   (A)   Any person desiring a permit to install or replace any culvert shall file an application therefor with the Village Clerk upon a form to be provided for that purpose.
   (B)   The application and the permit issued pursuant thereto shall be on the same form which shall be substantially as outlined in Appendix B at the end of this chapter.
