Every person desiring a taxicab license shall make application for the license to the Clerk-Treasurer upon a form furnished by the Clerk-Treasurer. On the application the applicant shall give the following information:
(A) Name.
(B) Whether a corporation.
(C) The names of all corporate officers.
(D) Their addresses.
(E) The corporate business address.
(F) Evidence of financial responsibility.
(G) Whether convicted of a felony.
(H) A form showing the approval of the vehicle to be used issued by the Chief of Police.
