Any person desiring a taxicab driving permit shall file an application therefor with the Chief of Police upon a blank furnished by the city for such purpose and shall, among other things, state: name; address; physical description; former employer; court record and state chauffeur’s license number. Such application shall be signed and sworn to by the applicant. A physical performed by a qualified doctor is also required.
