3-4-3: APPLICATION FOR LICENSE:
An application for said license shall be made in writing to the village manager on provided forms. The applicant shall attach to the application a copy of the certificate of registration for the location for which a license is sought, and shall maintain said certificate in good standing with the department of professional regulation.
