Every application for a school of massage shall be accompanied by:
(A) Proof of approval pursuant to Cal. Education Code §§ 94900 or 94901;
(B) A statement of the educational and experience qualifications, and the names and residence addresses of all directors, administrators, and instructors;
(C) A copy of the course outline, schedule of tuition, fees and other charges, regulations pertaining to tardiness and absence grading policy, and rules of operation and conduct;
(D) A school of massage permit shall not be issued for the same location as a massage/bodywork establishment.
