§ 33.063 HEALTH DEPARTMENT FEES.
   (A)   Definitions. For the purpose of this section, the following definitions shall apply unless the context clearly indicates or requires a different meaning.
      BOARD OF HEALTH. The County Board of Health as provided for in I.C. 16-20-2-1 et seq.
      HEALTH DEPARTMENT. The County Health Department.
      PERSON. Any association, company, corporation, firm, individual or partnership.
   (B)   Collection of fees. The Health Department shall collect the fees for all services it provides in accordance with this section.
   (C)   Fees for services. The Board of Health shall charge a fee for providing any person with the services listed in division (E) below.
   (D)   Collection of fees, accounting and disposition.
      (1)   Collection of fees. The Health Department shall collect the fees established by this section in accordance with division (E) below.
      (2)   Accounting for fees. All fees collected by the Health Department shall be accounted for in detail for each program service area.
      (3)   Disposition of fees. All fees collected by the Health Department pursuant to this section shall be transferred to the County Health Fund. The moneys collected in accordance with this section shall be used only for the maintenance or future expansion of the specific program service areas from which they are derived.
      (4)   Fees for other county health jurisdictions. Fees collected for health services and records provided to persons in other county health jurisdictions and involving payment from county tax revenues shall be collected in accordance with agreements entered into pursuant to I.C. 16-20-1-8.
   (E)   Fee schedule. The following fee schedule is established and approved for the services and records provided by the Health Department:
