139.07 WRITTEN REPRIMAND.
   A written reprimand is issued after the employee receives both a verbal and written warning which relates to the same or similar unsafe work behavior. The conditions which apply to both verbal and written warnings must apply: that the supervisor witness or be informed of the unsafe work behavior; that the supervisor verbally details the unsafe work behavior to the employee; that the supervisor elaborates upon the consequences of such behavior; that the supervisor intends to write a reprimand. The written reprimand shall be forwarded to the Director of Safety for inclusion in the employee's employment record.
