In the event that a supervisor witnesses or is informed of an unsafe work behavior for the second time, the supervisor shall detail the behavior, elaborate upon the consequences and inform the employee of his or her intent to issue a written warning. The written warning shall include the employee's name, the date, the nature of the unsafe work behavior, the date of previous verbal warnings and any other pertinent information and shall be placed in the employee's personnel file.
