139.04 FIRST VERBAL WARNING.
   Supervisors, once informed of an unsafe work behavior, shall attempt to observe the behavior first-hand. If the unsafe behavior is observable, then the supervisor shall warn the employee verbally by detailing the behavior and elaborating upon its consequences. In every instance, the supervisor shall make a written notation of the warning. In the case of a verbal warning, the date, employee name and general nature of the infraction shall be noted. If the work behavior is not observable, the supervisor shall make a general statement or caution to the employee regarding the nature of the work behavior and watch for similar behavior in the future without recording the incident. The supervisor may be the initial observer of unsafe work behaviors. In such an instance, the supervisor shall issue the standard verbal warning and place a written notation of such warning in the employee's personnel file.
