(a) The Director of Public Safety shall be responsible for the preparation of checklists. Checklists, developed for use by department personnel, seek to determine potential sources of occupational injuries and accidents. Through cooperative discussion, the department heads and the Director of Safety set schedules for regular, required completion of Department Core checklists. Each department head shall designate an employee to complete core checklists. Departments shall include Divisions, such as the Division of Police and Division of Fire with the head or chief of each division having the same responsibility as a department head.
(b) Upon completion of core check procedures, each department head will review the checklists to determine if any safety-related precautions need improvement or implementation. Each department head will initiate efforts to rectify the current deficiency and record in writing the actions taken to effect compliance with the City's Core Safety checklists. Within one week of the completion date of the core checks, the department head will forward original checklists and written corrective action reports to the Director of Safety.
(c) The Director of Safety shall maintain files which contain completed core checklists forwarded from department heads according to predetermined schedules. Additionally, the Director of Safety shall require reports on efforts to comply with safety precaution deficiencies exposed by core check procedures until a deficiency has been rectified.
(d) Each department head shall develop the written means for employees to report potential worksite or work practice hazards. This additional source of input shall be used to revise each Department Core checklist periodically. Revision of core checklists shall be the responsibility of Department Safety Committees and shall be accomplished annually from input resulting from bi-annual safety checks.
