The following information shall also be required.
(a) A statement of the proposed use of the building sites or lots so as to reveal the type of residential development with the number of dwelling units; the type and extent of business, industry, institution or other structures and their effect on traffic flow through streets, public service facilities, fire or health hazards, or congestion of population;
(b) Any existing or proposed covenants and restrictions;
(c) The source of water supply and provisions for sewage disposal and storm drainage; and
(d) An indication of all parcels of land intended to be dedicated or temporarily reserved for public use, or to be reserved in the deeds for the common use of property owners in the subdivision, with the purpose, conditions or limitations of such reservations.
