791.04 REGISTRATION OF ALARM INSTALLER PERSONNEL.
   Each of the alarm agents of an alarm business shall submit to the City Police Department the following information upon forms provided by the Police Department:
   (a)    The agent's or employee's name, current residential address and age.
   (b)    The number of the agent's or employee's current Ohio driver's license.
   (c)    The agent's or employee's fingerprint record.
   (d)    A recent photograph of such agent or employee.
   (e)    A copy of any alarm installer agent permit issued to the agent or employee by any other municipality or governmental agency.
   (f)    Three character references.
   (g)    The agent's or employee’s employers during the past three years.
   (h)    Any criminal record convictions.
