757.02 APPLICATION REQUIREMENTS. 
   Such report shall contain the name and address of the person or the names and the addresses of the person or persons representing the group or association, the location where the picnic, clambake or other gathering is proposed to be held, the date and hours during which the same is expected to continue and the number of persons expected or invited to attend. Such report shall also contain full information as to whether or not any alcoholic beverage is expected to be sold or dispensed on the premises, where and when the picnic, clambake or other gathering is proposed to be held.
