The license required in Section 713.02 shall be issued by the Mayor or a person acting and authorized to act on his behalf upon the payment of the license fee and the submission of an application containing the following information:
(a) The name and permanent address of the applicant;
(b) A description of the property to be sold and an estimate of its sale value;
(c) The name and address of all persons who will be associated or engaged in such public sale; and
(d) The name and address of the party for whom the sale is being held.
