The Superintendent may, at the end of the time limit allowed in the original notice of violation, allow an additional 60 days for the satisfactory correction of the violation, if the following conditions exist:
(A) This is the first such violation concerning this particular discharge or substance for this user;
(B) The user is making a good faith effort to remedy the violation;
(C) The user submits progress reports containing information specified by the Superintendent every 30 days;
(D) There is a strong likelihood that the violation will be eliminated at the end of the 60-day period.
