No person, firm or corporation, shall operate a waste water treatment plant without first obtaining the following:
(a) A waste water treatment plant operator's license from the State;
(b) An operator's license from the Village. Such license shall be issued by the Director of Public Service after an application has been completed on such forms as are furnished by the Director of Public Service and approved by Council.
Operators licensed by the State of Ohio shall place on file with the Mayor a certified or photostatic copy of such license.
Any operator who fails to maintain or repair a waste water treatment plant within five days after notice to maintain or repair as required by Section 927.04, shall forfeit the license issued by the Village.
