Every applicant desiring to obtain any exhibitors license or licenses shall file with the Safety Director a written application stating the full name and address, including street and number of the applicant, or if more than one person, or if an association or firm, the full names of all parties interested, and their addresses including street and number. If the applicant is a club, society or corporation, the application shall contain a complete list of the officers of such club, society or corporation, with their names and addresses, including street and number, and shall also give the state in which the club, society or corporation is organized, and the names and addresses of one or more persons whom the club, society or corporation desires to designate as its manager or managers, person or persons in charge. The applicant shall also set forth the name and address of the statutory agent when applicable. The application must also state the following:
(a) The number of coin-operated machines to be exhibited;
(b) The address at which the devices are to be maintained;
(c) The serial numbers, manufacturer and name of each device;
(d) That the applicant or manager meets the qualifications as set forth in Section 781.03;
(e) Whether the applicant has ever been engaged in operating an amusement arcade and when, where and how long in each place within five years preceding the date of application.
