3-4-1: APPLICATION FOR LICENSE:

Original application for a mobile home park license shall be filed with the village clerk, shall be in writing, signed by the applicant and shall contain the following:

A. The name and address of the applicant.

B. The location and legal description of the mobile home park.

C. The complete plan of the park. (1968 Code § 2.09)
