(A) The Health Officer shall issue and serve upon the permittee a notice setting forth in clear and concise language the actions or omissions with which the permittee is charged and informing him or her to his or her right of a hearing.
(B) At any time within a 15-day period after service of the notice, the permittee may request to show cause why his or her permit should not be suspended. The failure to request a hearing within 15 days shall be determined a waiver of the hearing.
