§ 4.68 PROOF OF VACCINATION A CONDITION OF ISSUANCE; VETERINARIANS TO FURNISH PROOF UPON INOCULATION.
   (A)   No license required by this article shall be issued for any dog unless the dog is effectively vaccinated with canine anti-rabies vaccine. As a condition of issuance of a license, the licensing agency shall require from each owner proof of currently effective vaccination of each dog sought to be licensed, the proof to be in the form of a veterinarian's certificate.
   (B)   A veterinarian administering an anti-rabies vaccination shall provide the owner of a dog so vaccinated with a certificate, indicating the date of vaccination and description of the dog vaccinated.
