1458.04 PERMIT APPLICATION, ISSUANCE AND REVOCATION; ILLUMINATED AND ELECTRICAL SIGNS.
   (a)   An application for a permit to erect a sign shall be made upon a blank provided by the Chief Building Official and shall contain or have attached thereto the following information:
      (1)   The name, address and telephone number of the applicant;
      (2)   The location of the building, structure or lot to which or upon which the sign or other advertising structure is to be attached or erected;
      (3)   The position of the sign or other advertising structure in relation to nearby buildings or structures;
      (4)   Two blueprints or ink drawings of the plans and specifications and the method of construction and attachment to the building or in the ground;
      (5)   A copy of stress sheets and calculations showing that the structure is designed for dead load and wind pressure in any direction in the amount required by this chapter and all other laws and ordinances of the City;
      (6)   The name of the person, firm, corporation or association erecting the structure;
      (7)   The written consent of the owner of the building, structure or land to which or on which the structure is to be erected;
      (8)   Any electrical permit required and issued for said sign;
      (9)   Such other information as the Chief Building Official shall require to show full compliance with this chapter and all other laws and ordinances of the City.
   Each applicant for a permit for a sign which will extend over public property more than fifteen inches, and, if the Chief Building Official believes that the public interest requires it, an applicant for a permit for any sign extending over public property less than fifteen inches, shall execute a continuing bond in a sum to be fixed by the Director of Public Service, but in no event to be less than one thousand dollars ($1,000), with an approved surety company as surety thereon, indemnifying the City against all loss, cost, damage or expense incurred or sustained by or recovered against the City by reason of the construction or maintenance of such sign. A liability insurance policy issued by an insurance company authorized to do business in the State conforming to this section may be filed in lieu of a bond.
   (b)   No sign shall be erected or attached to, suspended from or supported on a building or other structure until a permit for the same has been issued by the Chief Building Official. No permit for an illuminated sign shall be issued without the prior approval of the Electrical Inspector. Electrical signs shall be constructed and wired in accordance with the standards contained in the National Electrical Code.
   (c)   The Chief Building Official is hereby authorized to revoke any permit issued by him or her upon failure of the holder thereof to comply with the provisions of this chapter.
