1424.06 REQUIREMENTS FOR CERTIFICATION.
   (a)   All applicants must submit a Tax Registration Certificate issued by the City Income Tax Department.
      (1)    A Tax Registration Certificate shall be issued upon the applicant's ability to show that all income tax liabilities with the City have been paid or are in the process of being paid; and
      (2)   Applicant has filed all necessary current tax returns with the City.
   (b)   (1)   The application shall include a form requiring the following:
         A.   The name, address, business and home telephone numbers and Social Security number or Federal Tax Identification number of the applicant, type of ownership (sole proprietorship, partnership, corporation) and the responsible parties for legal notification and a copy of their drivers license or other identification approved by the Chief Building Official.
         B.   The name, address and Social Security number of each employee and subcontractor of the applicant who are currently working on any site in the City.
         C.   The name, address and Social Security number of each employee and subcontractor of the applicant who worked on any site in the City during the calendar year preceding the year of registration, provided that, in case of an applicant which files its application prior to January 31 of the year of registration, the Chief Building Official may issue a Certificate of Registration on condition. that the applicant submits the information no later than January 31 and if the information is not so submitted the Chief Building Official may suspend the conditional Certificate of Registration until such time as the information is submitted, upon giving written notice to the contractor.
         D.   A current Certificate of Competency from the State of Ohio must be submitted for all plumbing, H.V.A.C., sprinklers and electrical contractors.
         E.   A current license must be submitted for all asbestos, lead and radon abatement and testing contractors.
         F.   A certificate of insurance naming the City and its agents as additional insured parties in the minimum amounts of one hundred thousand dollars ($100,000.00) per person and three hundred thousand dollars ($300,000.00) per occurrence for bodily injury and one hundred thousand dollars ($100,000.00) per occurrence for property damage. The certificate of insurance shall also name the City Building Department as Certificate Holder.
      (2)   The Chief Building Official may accept other states' licenses at the official's discretion.
