§ 125.016 TAXICAB DRIVER’S LICENSE APPLICATION.
   A verified application for a taxicab driver’s license shall be made in writing to the Chief of Police, or his or her designate, upon terms prescribed and provided for the purpose by the Chief of Police. The application shall include at a minimum:
   (A)   Name. The full name of the applicant and any alias names;
   (B)   Address. The applicant’s residence address and residence addresses of the three years next preceding the application;
   (C)   Length of residence. The length of residence of the applicant in the city;
   (D)   Description. The applicant’s physical description;
   (E)   Birth date. The applicant’s date and place of birth;
   (F)   Citizenship. The applicant’s national citizenship;
   (G)   Prior employment. The places of applicant’s previous employment for the three years next preceding the application;
   (H)   Prior licenses. A statement of whether any taxicab driver’s license has been previously issued to the applicant by the city or other jurisdiction, and whether any license has ever been revoked or suspended;
   (I)   Accidents. A statement of the date and nature of all accidents and citations involving the applicant while driving a vehicle; and
   (J)   Waiver. A waiver authorizing the Chief of Police, or his or her designate, to investigate the applicant’s background.
