§ 94.05 APPLICATION.
   Applications for open burning permits shall be made to the Fire Chief in writing signed by the applicant, and shall contain the following statements and information:
   (A)   The name, age, address and telephone number of the applicant;
   (B)   The address of the burning site;
   (C)   The quantities and types of items to be burned;
   (D)   The methods or actions to be taken to control or extinguish the fire;
   (E)   Statement of why alternatives to open burning are not available;
   (F)   Proposed date of the open burning;
   (G)   Statement as to whether or not residents in areas near the burning site have been notified; and
   (H)   A sketch of the open burning site showing distances to the nearest residences, fire hydrants, streets or alleys, schools and any other structure of any kind which is near the site.
