Any owner, or agent thereof, of property for which a special use is proposed, shall make an application for a special use permit. The site plan application and the information required as stated in Section 1222.03 shall constitute the special use application. However, the following additional information shall be filed with the special use application:
(a) A list containing the names and addresses of all owners of property within and contiguous to, and directly across the street from the property in question as appearing on the County Auditor's current tax list or the County Treasurer's mailing list.
(b) A fee as established by Council. A site plan application fee is not required.
(c) A narrative addressing each of the applicable criteria contained in Section 1288.03.
