(a) Fees for water-line maintenance shall be as provided in the following table:

(b) The minimum monthly fee, as set forth in division (a) of this section, shall be based on the meter size installed to the customer.
(c) The minimum monthly fee, as set forth in division (a) of this section, shall remain in effect for each customer until one year after discontinuation of service.
