(A)
Any city official or employee holding a policy-making position that involves purchases, sale or lease of property, obtaining grants or loans, fiscal negotiations and authorizations, and other such responsibilities who shall have any private financial interest, directly or indirectly, in any contract or matter pending before or within the city government shall disclose such private interest to the Ethics Commission.
(B) Any city official or employee who may have a personal or financial benefit in any matter pending before the City Council shall disclose such private interest on the record of the City Council and shall disqualify himself or herself from participating in any decision or vote relating thereto. If the disclosure occurs in a legally conducted closed meeting the official or employee will leave the room.
(C) Any city official or employee who shall have a private interest, directly or indirectly, in any contract or matter pending before or within the City Council shall disclose such private interest to the Ethics Commission and to the City Council.
(D) Every city official or employee shall disclose, not later than the first business day in March on a form provided by the Ethics Commission, a statement of financial interests, which shall include:
(1) The name and address of employers from whom the officer or employee was paid salary and/or wages totaling more than $10,000 during the previous calendar year.
(2) The name and address of any legal entity on which the officer or employee serves as an officer or director as of December 31 of the reporting year.
(3) The name and address of business interests, real estate (other than his or her primary residence), rental property, and farms in which the officer or employee owns at least a 5% interest, such interest totaling more than $10,000, on December 31 of the reporting year.
(4) Each statement shall be signed and dated by the individual filing the statement of financial interest. Knowingly signing a fraudulent statement shall be a class A misdemeanor.
(E) Candidates for city office shall file disclosure forms as though they were city officials 30 days prior to the election; provided, however, candidates who have filed a form by reason of incumbency or employment within the past 12 months shall be exempt from this provision.
(F) No city official, employee or candidate for public office within the city shall be required to disclose his or her personal or family financial records, statements or documents of financial nature except as provided herein.
