Verified applications to register a business shall be made by the owner of the business, trade or profession, or by a designated agent, upon a form to be furnished by the city clerk. Such application shall contain the following information:
A. The business name, physical and mailing addresses, and phone number;
B. The full name, physical and mailing addresses, and phone number of the owner of the business, trade or profession;
C. The name and address of an agent of the business, trade or profession, who is designated by the owner as authorized to receive any notices permitted or required by law to be provided to the business, trade or profession;
D. The name, address, and phone number of an emergency contact person, who is designated by the owner to respond to emergencies at the business location (e.g., fire, burglaries, etc.);
E. A description of the nature and purpose of the business, trade or profession to be registered;
F. The signature of the owner of the business, trade or profession, or the owner's designated agent; and
G. If applicable, a copy of the business's Idaho State Tax Commission seller's permit shall be submitted with the registration application. (Ord. 535, 11-15-2010)
