An application for a parade permit shall be in writing on a form provided by the police department and shall be submitted to the chief of police or his designated representative. The application shall contain all information necessary to determine the name of the sponsoring organization, the number of entries in the parade, date and time of the parade, description of parade route, and any other matter or thing which may be required by the chief of police.
