(A) Notification requirements. Commercial lawn herbicide or lawn pesticide applicators shall provide any customer within the city limits receiving an herbicide or pesticide lawn treatment with the information listed in subsections (1) through (3) below:
(1) Date, time and address of property treated;
(2) Name, address, telephone number, state license number, and signature of the applicator;
(3) Name of the herbicide or pesticide, dosage used, and the location of the area treated.
(B) Record retention. Copies of customer notification shall be retained by the commercial lawn herbicide or lawn pesticide applicator for a period of two years after the date of treatment.
